Age Care Connections Website
The programs and social services offered by Aging Care Connections are provided to adults age 60 and older and their family members. Programs and services are designed to provide options for care to seniors in order to enhance their ability to remain as independent as possible in their own communities. Aging Care Connections employs professionally-trained staff who can assess the needs of older adults facing a change in health or functional ability on an individual basis. A case manager can offer information, assistance and referral to in-home services designed to address the needs identified. Programs and services can also provide support and assistance to family members caring for older adults. Caregivers can receive valuable information on a wide range of programs, services, support and resources currently available.
Coast2Coast Prescription Discount Card
Allows all Lyons Township residents, regardless of income, age, or health status to participate in the program and save on the cost of their medications. Even pets and other people living outside the Township are able to take part in the program. The Rx card will be available for residents at participating pharmacies, governmental offices, libraries and the Township General Assistance office. You can also print the Rx card, check your prescription prices and view the nearest participating locations on the Coast2Coast Website.Community Nurse Health Center

We serve as a community healthcare center for residents of the western suburbs of Chicago, focusing on those who have limited access to care. We value patient dignity, community collaboration and prevention and wellness by delivering coordinated and integrated health care services and education. Interfaith Community Partners is a non-profit organization that provides escorted transportation to older adults in the Lyons Township and surrounding communities. Our faithful volunteers provide rides to medical appointments, dialysis, and life-enhancing errands like the grocery store, hairdresser, and bank. We do not charge our clients for services and rely solely on private donations, grants, fundraisers, and local church support for funding.
With monies granted through Community Memorial Foundation, we developed a community calendar with programs for older adults such as education, exercise, spirituality, volunteer opportunities, and more. Please visit www.interfaithcommunitypartners.org and click on the calendar tab.

General Assistance is mandated by Illinois State Statutes. The program provides monthly financial service for people attempting to enter or re-enter the work force. Also, the program provides financial assistance to people who have a pending Social Security Disability application. This program is designed for people who have no income.

Service is provided for residents 60 years of age or older and/or handicapped persons. This service is provided for Lyons Township residents only for local shopping, doctor and dental appointments, social meetings etc. A transportation permit is required. Please view the Township of Lyons Website informational page on the Senior Transportation Program.
Transportation Fees: No charge to Senior Township Residents.
Service Boundaries: Service is available within the boundaries of Lyons Township and 2 miles beyond for doctor appointments only.

RESERVATION PROCEDURE: To reserve a ride, contact 708-354-0292 or 708-354-0293 between the hours of 8:00 am and 3:00 pm the day before your trip.
Senior Shopping Days
Special Shopping days where you can go to the grocery store of your choice. You may go grocery shopping only one day a week. This is to give everyone a chance to shop for their groceries. You are allowed only 3 bags/parcels.
North of the Canal: Tuesday or Thursday
South of the Canal: Wednesday or Friday
On these days, the drivers are permitted to help you on the bus with your groceries and bring them in as far as the curb.
